    Background: The second demonstration experiment of supporting elderly people going out with the Choisoko system was conducted. The first study showed that for women, friends, shopping, convenience, and events are factors that have the potential to be effective motivational factors for encouraging these women to go out. On the other hand, these factors did not lead to any behavioral change in men. Since there are approximately 15 million men over the age of 65 in Japan, behavioral changes in the entire elderly population will not occur without guidance for elderly men to go out. Methods: Sixteen elderly men and forty-seven elderly women participated. Interestingly, men are far more passionate about games than women. Therefore, we hypothesized that a preference for games could be a hint as to how we might encourage older men to go out. Then, a second demonstration experiment was conducted, and we analyzed the relationship between six game preferences and the frequency of going out. Results: Among gaming preferences, men with gaming preferences such as Philanthropists, Achievers, and Free Spirits showed a tendency to go out. Conclusions: These stimuli may have the potential to be factors that may encourage elderly men to go out.

    Maintaining a social environment that enables going out freely is important for older people and aids the prevention of frailty syndrome. However, losing a driver’s license can increase the long-term care needs of older people. Therefore, outing support systems are important. However, the utilization rate of these systems is currently relatively low. We conducted a demonstration experiment among older people aged 70 years and over, living in Iruma City, Saitama Japan, by implementing the Choisoko outing support system developed by Aisin Co., Ltd., in conjunction with an approach for improving motivation. Using this system, elderly people were able to go shopping freely whenever they chose, without a driver’s license. Participants in the demonstration experiment exhibited higher Functional Independence Measure scores after the intervention, irrespective of whether or not they used the Choisoko system. The number of uses per person increased over time, and the subjective well-being of Choisoko users improved. However, few male participants engaged with the system. Although improving motivation is important for inducing positive behaviors and enabling the elderly to go out, motivation-improving factors differ between men and women.

  • Socioeconomic status and the likelihood of informal care provision in Japan: An analysis considering survival probability of care recipients.

    Studies show that the burden of caregiving tends to fall on individuals of low socioeconomic status (SES); however, the association between SES and the likelihood of caregiving has not yet been established. We studied the relationship between SES and the likelihood of adults providing long-term care for their parents in Japan, where compulsory public long-term insurance has been implemented. We used the following six comprehensive measures of SES for the analysis: income, financial assets, expenditure, living conditions, housing conditions, and education. We found that for some SES measures the probability of care provision for parents was greater in higher SES categories than in the lowest category, although the results were not systematically related to the order of SES categories or consistent across SES measures. The results did not change even after the difference in the probability of parents’ survival according to SES was considered. Overall, we did not find evidence that individuals with lower SES were more likely to provide care to parents than higher-SES individuals. Although a negative association between SES and care burden has been repeatedly reported in terms of care intensity, the caregiving decision could be different in relation to SES. Further research is necessary to generalize the results.

  • Mass evacuation and increases in long-term care benefits: Lessons from the Fukushima nuclear disaster.

    BACKGROUND: Though mass evacuation may increase the need for long-term care (LTC) services, how the need for LTC services increases and how the public LTC system affects it is not well understood. We evaluated changes in public LTC benefits for the people living in the mandatory evacuation areas established after the 2011 Fukushima nuclear disaster and examined the roles of the universal LTC insurance system in Japan. METHODS: In order to evaluate the effect of the mandatory evacuation on LTC benefits, we examined the trends of LTC benefits in the Fukushima evacuation group and the nationwide non-evacuation group. We first decomposed per-elderly-individual benefits at the municipality level into the LTC certification rate and per-certified-individual benefits, and then implemented difference-in-differences analysis using these variables as outcomes. RESULTS: Per-elderly-individual benefits significantly increased from 2012 onward in the evacuation group, and this was explained by an increase in the certification rate rather than in per-certified-individual benefits. Increases in per-elderly-individual benefits and the certification rate in the post-disaster period were observed in all but the highest care level, and the corresponding outcomes for the highest care level decreased immediately after the disaster. We also found that the increase in the certification rate had been mostly realized by an increase in the number of certified individuals. CONCLUSIONS: The increase in LTC benefits can be associated with the impact of the increase in the number of people newly certified to receive LTC benefits after the mandatory evacuation. In order to cope with the increase in utilization of long-term care and associated costs after disasters in aging societies, both formal long-term care services and social support for informal care for evacuees should be considered important.

  • The Fund for Comprehensively Securing Regional Health and Long-term Care in Japan

    急速な少子高齢化に伴う医療・介護需要の一層の増大が見込まれるなか,「団塊の世代」が全て75 歳以上となる2025 年を1 つの目安として,「地域医療構想」や「地域包括ケアシステム」の構築,医療・介護人材の確保・養成の強化といった医療・介護の提供体制改革が進められている。

    こうした改革を支える新たな財政支援制度として,2014 年度に各都道府県に設置されたのが「地域医療介護総合確保基金」である。この基金は,都道府県が医療・介護の総合的な確保のために実施する事業の経費を支弁するために造成され,医療分として約904 億円,介護分として約724 億円が,それぞれ毎年積み立てられている。しかし,医療分の対象に,病床の機能分化・連携に関する事業だけでなく「医療従事者の確保に関する事業」も加えられ,医療従事者の確保・養成に関わる多数の国庫補助事業が,新しい基金で引き続き実施可能であるとして廃止されたことは,この新しい基金の性格を曖昧なものにしてしまっている。

    本稿では,このような現状を踏まえ,各都道府県が公表している事業計画を分析し,次の2 つの知見を得た。第一に,医療分のみが実施された2014 年度計画では,看護師確保対策に重点的な予算配分がなされ,医師確保対策の約2 倍に当たる26.0%の予算が充てられていた。第二に,国庫補助からの継続事業が識別可能な10 都府県分の集計では,2014 年度計画分予算の34.2%が継続事業に割り当てられていた。

    このように,継続事業への予算配分は全体の約3 分の1 を占めており,国が国庫補助事業から基金に振り替えられた事業の規模として事前に説明した額(274 億円,904 億円の約30%)が実際に確保された。しかし,2015 年度以降は医療分予算を病床の機能分化・連携に関する事業に重点配分していく方針が示され,国庫補助継続事業は事実上の縮小・廃止を迫られている。国は,地域医療介護総合確保基金で対応可能であるとして国庫補助の廃止された事業の取り扱いについて明確な説明が求められる。

    国民健康保険料(税)の深刻な滞納者への資格証明書交付は受診抑制に繋がるおそれが指摘されているが, その交付基準は市町村によって異なる。そこで本稿では, 資格証明書交付世帯割合の決定要因を分析し, 保険料収納率を統御したうえで, 市町村の財政状況による有意な影響があることを観察した。財政状況が厳しい自治体ほど, 資格証明書の積極的な交付により医療アクセスの阻害が生じている可能性があり, そのような問題の生じない形で資格証の運用基準の統一を図る必要がある。
    While the risk of medical restraints in issuing eligibility certification cards to serious delinquents of national health insurance (tax) has been highlighted, the eligibility criteria differs from one municipality to another.
    In this regard, this study analyzes the determinants of eligibility certification cards issued per household ratios, examining relevant influences from local public finance conditions, after controlling for insurance fee levy rates.
    The more stringent the municipality finance conditions, the greater is the possibility of hurdles to access to health care due to a possibly proactive issuing of eligibility certification cards; an attempt at unifying the eligibility certification management standards is necessary to avoid these types of problems.

  • An Analysis of the Medical Assistance Costs in Japan

    In recent years, the amount of medical assistance has rapidly increased, and the government has proposed policies to reduce outlays and encourage the discharge of long-term inpatients. However, the amount of medical assistance per capita has tended to decrease, a trend that has not been adequately studied. This article analyzes the determinants of levels of medical assistance by using panel-data at the prefecture level. The results show that rising rates of longterm psychiatric inpatients have increased the amount of the medical assistance per capita. It suggests that policies to encourage the discharge of long-term psychiatric inpatients have been effective reducing outlays on medical assistance. However, we need to carefully examine from medical and social perspectives in deciding whether this policy is truly beneficial.

    概要
    医育機関としての旭川医科大学は初期臨床研修及び後期臨床研修を行い、各専門診療科において専門性の高い教育を行いつつも、地域の二次医療機関からの要請により医師を派遣している。旭川医科大学が置かれている状況を既存統計から明らかにした上で、引き続く高齢化に対して地域の(二次)医療機関が対応できない場合には、医学教育の直面する困難がさらに深刻になる可能性があることを示した。循環呼吸医療再生フロンティア講座で実施している地域医療実習をさらに推進するほか、地域の医療機関との教育面での連携を強化する必要があると考えられる。

    本稿は, 2009年12月に実施されたひとり親世帯就労促進費廃止と母子加算復活が, 被保護母子世帯の母親の就業率を引き下げた可能性について, 厚生労働省「社会保障生計調査(2009年度)」の個票データを用いて, 明らかにした。ただし制度変更が就業率に負の影響を与えたとしても, 社会厚生水準の観点からは問題とはならない可能性もあり, 就業率の下落(あるいは上昇)が社会的に望ましいかどうかの価値判断にはなお慎重な留保が必要である。
    This study clarifies the possibility of decreasing employment rates among single mothers receiving public assistance after the repeal of the Fund for Promotion of Employment for Single-Parent Households and the return of the Maternal Additional Assistance Premium, using the individual data of the Ministry of Health, Labor and Welfare's "Survey on Living by Social Security" (Fiscal Year 2009).
    However, even if system reforms negatively impacted employment rates, a possibility exists that, from a social welfare perspective, this is not becoming a problem and therefore it is critical to carefully withhold reservation toward value judgments regarding whether a decrease (or increase) in employment rates is socially desirable.

    Universal health coverage (UHC) assures the access to the health care services for all of residents with various life styles/employment status. It is very important to keep the function of UHC even in the population decreasing super-aged society. This study considered the health insurance system, function of health insurers and the way of cost sharing of health care, with empirical results using individual data.
    Empirical studies in this research project showed the results as follows; 1)Burden of social insurance premium was lowered for lower income classes this 10 years. 2)However there was exceptional individuals in those income class with very high burden of social insurance premium. 3)Lowered premium burden did not necessarily improve the rate of unpaid social insurance premium. 4)In view of service utilization, upper limitation of coinsurance rate keeps the assurrance of access to the health care services.

    Conventional explanations on the Demographic Transition (DT) omits impacts of structural changes of population. The simulation analyses on DT, however, indicated that the changes had significant effects on crude death, and birth rates through the population momentum thus on the process itself when the transition was quick. The structural changes should be an essential part of the process and be included in the theory. As for the empirical findings, the childless rate has been increasing (even ever married) in Japan and the childless orientation accompanying late marriage and unmarried / divorce is currently prevailing as well. Findings from careful examination of the low fertility and the policy measures in Japan, suggest difficultly “to correct” the fertility situation by a single specific measure. Instead promoting comprehensive measures toward life and family formation with the life-course perspectives. These are all unique and creative viewpoints from the current study project.

    A major contribution of this project is to construct the micro-based data which consists of both health care (HC) and long term care (LTC) use for an elderly person. This data makes the inter-temporal use of both care traceable for each individual, even if a person transfers National Health Insurance to the latter-stage elderly healthcare system when he/she become 75 years old. Also, using this data, we could quantitatively clarify the relation between the use and expenditure of HC and LTC, the effects of the transfer of type of health insurance on HC expenditure, and the relation between HC expenditure and the insurance premium evasion, etc.
    Moreover, we conduct the inquiring survey (interview, participant observation and questionnaire) from interdisciplinary point of views of sociologists and economists in our project. Using this survey, we could verify socio-economic characteristics and health status of elderly person within the community who participate into the salon activities.
